Self-care and a good doctor are the ABC of health of the modern woman

21 June 2021 г.

Забота о себе и хороший врач — азбука здоровья современной женщины
Despite the fact that we live in the era of emancipation and gender differences are erased in many areas, one cannot do without woman’s doctors in medicine. Gynecology, women's health clinic - these terms are unlikely to disappear from our language. Lina Petrovna Gurova, obstetrician-gynecologist of the Research Institute of Medical Problems of the North, one of the institutes of the Federal Research Center "KSC SB RAS", tells about the achievements of modern gynecology, competent anti-aging and women's health problems.

Today in Siberia women have such opportunities in the field of medicine, which were not available a few years ago. But along with technological progress, it is important to change your attitude to health, and to take a responsible attitude towards oneself. The first step to taking care of oneself is, of course, a physical examination. What every woman at any age should do is visit a gynecologist at least once a year. If you have any complaints, you should contact the specialist immediately.

For a young woman, a gynecological ultrasound scan, examination of the mammary glands, colposcopy, a smear for cancer cells of the cervix are the minimum. But this minimum can sometimes save lives. These are the localizations where pathologies can be seen very quickly. Even if doctors find malignant changes, then this is a timely diagnosis, an opportunity to cure it, like any other disease. When a patient arrives with an advanced stage, it is already impossible to do anything. Therefore, if there are complaints, one needs to look for a doctor, to ask questions.

At what age is hormonal support recommended?
Each age should have its own hormonal support. At a young age it is contraception or treatment for hormonal disorders. The doctor is a specialist in this area and will help with these issues.
In adulthood, it is menopausal therapy. The biggest problem during this period is calcium loss. Estrogens help retain calcium in the body of a woman. When there is no estrogen, calcium is washed out. This must be remembered. Bones can be saved by starting therapy a little earlier than menopause. The therapy should be started 2-3 years before the onset of menopause. This is much more effective otherwise it can be too late.

Moreover, not only women but also men suffer from calcium deficiency. Now the role of vitamin D is widely discussed. This is not a fashion, we really need it both for the absorption of calcium and for the biochemical processes in the body to live healthily. With age, there arises a problem with vitamin D. It is necessary to monitor laboratory indicators of vitamin D levels from premenopause, especially in our region. If there is a decline, measures must be taken. It is important to eat fatty fish and get Omega-3 fatty acids from the diet.

In general, a wide range of anti-aging measures are now available. The whole systems of support of our organism have been developed, which help to be in a state of comfortable living. It must be remembered that the health of the working people is in the hands of the working people themselves. A doctor can only help from the professional viewpoint. It is the responsibility of a person to take care of his health. Even under the law of the Russian Federation, we are obliged to do this.

What problems can be solved through surgery today?

In terms of laparoscopy, a modern method of surgery, in which operations on internal organs are performed through small holes, Russian, and, in particular, Krasnoyarsk doctors have now stepped very far. It is worth saying many thanks to Leila Vladimirovna Adomyan for this. She brought this technology to Russia. Leila Vladimirovna is a Soviet and Armenian obstetrician-gynecologist, Academician of RAS, Honored Scientist of the Russian Federation, laureate of the RF Government Prize, Doctor of Medical Sciences, Professor, Chief Obstetrician-Gynecologist of Russia. She is the head of the Department of Operative Gynecology at the V. I. Kulakov National Medical Research Center for Obstetrics, Gynecology and Perinatology. She did a lot for the health of our women. Leila Vladimirovna is the author of 17 inventions and numerous works in the field of gynecology. She masters all types of traditional gynecological operations. She is the president of the Society for Reproductive Medicine and Surgery and the Russian Association of Endometriosis, and vice-president of the National Association of Gynecologists-Endoscopists.

Laparoscopy means fewer adhesions, minimal blood loss, and faster recovery. Nowadays there are completely different suture materials, the suture technique is such that the patient is allowed to sit almost immediately after the operation. After a month, one can live a normal life. The most important thing is that a woman must change, reestimate her lifestyle, and take care of her health.

In each case, we choose the best treatment option. We combine techniques, laparoscopy is suitable for some operations, sometimes we use traditional methods. There are quite "simple" operations, and there are complex ones, such as prolapse and prolapse of the female genital organs. Unfortunately, this is a fairly common problem in women in adulthood or old age, for example, after several traumatic childbirths in their youth. Sometimes women live their problem for years because of fear or other reasons. Therefore, I would like to tell you more about it.

There are various techniques. Conservative methods of treatment aimed at strengthening the pelvic floor muscles include special exercises such as Kegel exercises and others, physiotherapy, and myostimulation. Typically, these methods help women recover immediately after giving birth. When during childbirth there is an injury to muscles and connective tissues, ligaments, it is easier to restore the muscles. Basically, prolapse and prolapse are associated with injury to muscles, ligaments and connective tissue-fascia during childbirth. Muscles can be repaired, but the fascia problem, unfortunately, cannot be easily solved.

Therefore, in order to somehow strengthen them, mesh implants are used in gynecological operations. In private clinics, this is an expensive operation and not every woman can pay for it, especially in old age, when the main income is a pension. But there is an opportunity to get such an implant on a quota. Quotas are not issued to all clinics, but the Research Institute of Medical Problems of the North of the Krasnoyarsk Scientific Center has such quotas.

Of course, we also carry out many other operations, including plastic surgery of the genitals. Each operation has its own risks, but for a doctor who masters the technique, it is no different from other methods.

As a rule, in large clinics, such as the Research Institute of Medical Problems of the North, emergency medical care, Regional Hospital etc, use modern techniques and such operations are performed. Our institution does a certain amount of such operations based on the compulsory medical insurance system, especially for aged women.

What is important to know about operations only with your own tissues? There is a certain percentage of relapses. Women must understand that they need to take care of themselves. I talk to everyone both before and after the operation. It is not easy to identify connective tissue dysplasia. We can assume, by indirect evidence, that the person clearly has connective tissue dysplasia.

In Siberia, such opportunities appeared not so long ago. There were no implants, no quotas for operations. It is very bad that not all gynecologists are well aware of this problem. Why are patients left without medical care in this situation? Because the doctor at the place of residence believes that urinary incontinence, organ prolapse is the norm for older women. Some patients start looking for information on their own. They learn that they are not the only ones who have such a problem, that there are methods, surgical treatment, rehabilitation of patients after operations. At the same time, no matter how wonderful the operation is, the patient must understand the extent of one’s responsibility to oneself.
